1. Purpose of the K40 subject
The purpose of the K40 subject is to learn how to work with medical records, how a department runs, selected manual skills and communication with patients and colleagues. The purpose is not to deepen your knowledge of the chosen field. Great emphasis is therefore placed on preparing medical documentation, and taking part in this work is the priority of the placement.
2. Choose the format
The K40 Clinical Practice can be completed in one of the following ways:
- 8 weeks in one clinical field (with an inpatient department) at one department.
- 4 weeks in one clinical field and 4 weeks in the same field at another department.
- 4 weeks in one clinical field and 4 weeks in another clinical field.
- 4 weeks in a clinical field at an inpatient department and 4 weeks in an outpatient or laboratory field (e.g. imaging methods, general practice, etc.).

- If you choose a placement in Anaesthesiology and Resuscitation (ARO) as your only clinical field, you must make sure you will be able to write admission, discharge or transfer reports. If the department does not allow this, ARO cannot be recognised as a clinical field and the placement must therefore be combined with a clinical field where this is possible.
- Two outpatient or laboratory fields cannot be combined. For example, a combination of radiology and general practice is not permitted.
- The placement cannot be completed at a nuclear medicine department or with an emergency medical service ambulance crew (RZP).
3. Choose a department
The placement can be completed at departments of the Second Faculty of Medicine, Charles University, or at another approved department.
Outside the departments of the Second Faculty of Medicine, it can be completed at:
- a clinical field with an inpatient department at a district, regional or university hospital
(list of contracted facilities), - an outpatient department, including day care or day surgery units.
K40 may also include a placement within the Erasmus programme.
4. Arrange agreements and paperwork
Contact the Study Department in good time to arrange all the necessary agreements and administrative requirements.
5. Prepare the placement documentation
Detailed requirements for obtaining credit are given in SIS. The documents may be written in Czech, English or another language. Translation into Czech is not required for placements completed abroad.
For the entire placement, the student must submit:
- 6 admission reports,
- 6 discharge or transfer reports,
- 6 progress notes,
- 1 case report prepared according to the instructions,
- 1 placement report,
- a list of procedures performed for each department attended.
If the placement is split between two inpatient departments, a total of 6 admission reports, 6 discharge reports and 6 progress notes is submitted. They may be divided between the departments in any combination. However, a separate list of procedures performed is submitted for each department.
If the placement is split between an inpatient and an outpatient (or laboratory) department, the following is submitted:
- 6 admission reports,
- 6 discharge reports,
- 6 progress notes from the inpatient department,
- plus 6 outpatient or laboratory reports.
The documentation requirements for the inpatient department remain the same even if the inpatient part of the placement is shortened to four weeks.
6. Anonymise the documentation
The student is responsible for anonymising the medical documentation.
Before submission, all of the patient's personal data must be removed from the documents or covered so that they are illegible, in particular:
- first name and surname,
- date of birth,
- personal identification number,
- address,
- contact details.
Anonymisation can be done, for example, by blacking out the data with an opaque marker, covering it up, or making a copy of the document with the personal data covered beforehand.
7. Submit the documents
Submit all documents required for obtaining credit to the Study Department in printed form, bound together as a single set (e.g. in a folder or binder).
- Documents cannot be sent to the Study Department by email.
The deadline for submission is no later than one week after the end of the placement. Students who complete the placement during the summer months or as part of a stay abroad submit the documents immediately after returning to the Faculty. Students who have K40 split into two blocks submit the documentation only after completing the second block.
Each document must state:
- the student's name,
- study group,
- the year in which the placement was completed.
Only if the placement report is prepared as a communication output is it submitted electronically. Communication outputs must be requested in advance.
Important notes
- For the purposes of K40, a general practitioner does not need to be accredited for specialist training.
- Once a placement at a department of the Second Faculty of Medicine has been assigned, it is not usually possible to change it during the academic year.
- Forms for the list of procedures performed, by specialisation, are available in the Study Forms (Year 6) section.
- If no form is available for the given field, the student completes the form for the umbrella field (e.g. internal medicine for diabetology, surgery for plastic surgery, etc.)
