Entrance Procedure

Application Form

The on-line application is valid only after being

  1. saved
  2. confirmed
  3. printed in PDF (Czech and unofficial English version)
  4. dated
  5. signed
  6. sent (both versions) to the Study Department (address: 2nd Faculty of Medicine, Study Dept., V Úvalu 84, 150 06 Praha 5) with
    • CV
    • Original of doctor´s confirmation of fitness to undertake studies (please confirm directly in PDF application form or on a separate paper)
    • Applicants who are not 18 years old must include a power of attorney from their parents with permission to take part in entrance examinations
       

Verified photocopies of your General Certificate of Secondary Education (High-School Education) in English will be required on the date of the entrance exam.
